First, you need to know if it's a Long style clutch with the three groups of two close together bolts or a Borg & Beck clutch with six equidistant bolts. It's likely a 10-inch Long type but it depends on when the last time it had a replacement. The more modern diaphragm type clutches generally have the Borg & Beck style equally spaced bolts. The flywheel can be indexed and re-drilled for any clutch that is sold as a Y-block replacement if it doesn't match the OEM bolt pattern. The old Long types with the 10-inch 1-1/8" 10-spline clutch cover and disc are getting harder to source and the Borg & Beck are even harder.
Once you know what pattern and size clutch it has, you should be able to either find a clutch overhaul facility that can rebuild it or just order a modern replacement through Y-block sources.
