Head remover question

I am building a head remover. After reading 2 threads on how to use it I have questions. On thread says to tighten the spark plug adapter to remove head. Another thread says to tighten the 5 bolts over the studs to remove head. Only way I can see this would work would to have threads in the plate. If you tighten the bolts in spark plug adapter, how do you keep the bolt from turning in the adapter?
