Looks to me like the fan hub rode up over the key when being installed.I've seen that done where the owner said it was fine for a few years,then suddenly the fan was loose or cocked off at an angle like yours.I think they can be tightened crooked,but look OK from the side.After the shaft/hub/key wears everything loosens up.I've also seen them like that where the owner tries to get the fan off by prying on the pulley.If you can get a fan off an A by pry on the pulley without damaging anything then the fan was too loose to begin with.
