Use the alternator and an inverter. Simple, effective, and safe.
^This.
I'm in Clinical Engineering at our local hospital.
A good strong alternator and a 12V conversion is probably the safest and most afordable option.
While and Oxygen Concentrator is quite a simple machine, I'd get a PURE SINE wave invertor rated a good margin above the draw of the O2 Concentrator. If it's not PURE sine wave, you run the risk of blowing the power supply or burning out windings. Most of the power used by the O2 Concentrator is for the compressor inside. You'll require some VERY heavy wiring to the invertor too.
Someone tried using an invertor that wasn't pure sine wave in one of our helicopters, and did about $25,000 of damage to our equipment and caused a baby incubator to catch fire! You sure don't want that to happen inside a car with an Oxygen enriched cabin!
We don't have any 12V versions, so I can't make any comment about them, sorry.
