The ones most folks tend to miss are those all the way in the back of the body at the rear most frame cross member. Don't ask me how I know this :). Having taken three of these bodies off their frames, I probably have some pictures of some of these bolt locations I could post if I could find them in my files.
 
Roy Nacewicz's body to frame bolt kit includes a location drawing with the styles and sizes specified for each location.
